Portal Tip 1: How to View Documents

  1. Log on to the portal and click on “My Health”.

  1. Click on the “Documents” tab.

  1. A list of your documents will appear. To see one click on within the row.
  2. To print the document select
  3. To fax the document select

  1. Fill in who it is going to and the fax number.
  2. Click
  3. You can also email the document by clicking on

  1. Type in the email address.
  2. Click

Portal Tip 2: Viewing Information in Your Chart

  1. Log on to the portal and click on “My Health”.

  1. Click on the “Chart” Tab.

  1. You can sort information by date or provider by populating the “Sort By:” box.

  1. To view an item, click on the underlined name of the document you would like to open.

  1. To print the document select

  1. Fill in who it is going to and the fax number. Click
  2. You can also email the document by clicking on

  1. Type in the email address. Click

Portal Tip 3: Update Demographic Information

  1. Log on to the portal and click on “My Info”.

  1. Click on the “General” tab.

  1. In this section you can update you name, address, contact information, and social information.
  2. To update any information, simply click on the box you would like to update and begin typing in your new information.
  3. Once you have updated your information click on the top part of the screen.
  4. To update your emergency contact information, click on the “Emergency Contact” tab.

  1. In this section you can enter your emergency contacts name, address, contact information, and how you know them.
  2. Once you have updated your information click on the top part of the screen.
  3. To update the person responsible for your account click on the “Responsible Party” tab.

  1. Enter in the person’s name, relationship to you, their date of birth, address and contact information.
  2. Once you have updated your information click on the top part of the screen.
  3. To update your insurance information click on the “Insurance” tab.

  1. Enter the information in the corresponding boxes.
  2. To add a secondary insurance click on the “Secondary Insurance” tab.

  1. Enter the information in the corresponding boxes.
  2. For a 3rd insurance click on the “Tertiary Insurance” tab.

  1. Once you have updated your information click on the top part of the screen.

Please note this information does not transfer over to your health record. To have this changed you must contact your provider.
