Portal Tip 1: How to View Your Vitals

  1. Log on to the portal and click on “My Health”.

  1. Click on the “Vitals” tab.

  1. To view a vital click on in the row of the vital you wish to see.
  2. A box will appear like the one below.

  1. Click to get out of the box.

Portal Tip 2: How to Add a Vital

  1. Log on to the portal and click on “My Health”.

  1. Click on the “Vitals” Tab.

  1. To add a vital click
  2. Select the vital you wish to add.

  1. Enter the date it was taken.
  2. In the “Value” box, record what the result was.
  3. Enter any additional comments in the “Comments” box.
  4. Click
  5. A box will pop up asking you to confirm the information. Select yes or no.

Please note that deleting/adding information does not transfer over to your health record. To have this changed you must contact your provider.

Portal Tip 3: Viewing Vitals as a Chart

  1. Log on to the portal and click on “My Health”.

  1. Click on the “Vitals” tab.

  1. Click on in the row of the item you would like to see charted.
  2. A graph outlining that vital will pop up. See example below.

  1. If an item has that means the information is not available in graph form.

The My Charts App must be activated. See tips from week 12/27/16 to see how.
