Before You Start

Please have the following ready:

Your Form

Obtain your form from your employer, disability carrier, insurance company, government agency, or other requesting organization. New York Spine & Wellness Center and MedRecs Management do NOT provide the forms.

Your Provider’s Name

You’ll be asked to identify your New York Spine & Wellness Center Nurse Practitioner/Physician’s Assistant.

A Credit or Debit Card

Payment is collected for form completion.

Both FMLA and Disability Paperwork

These must be submitted as two separate requests.

Fees & Processing Time

New Form – $30

Standard processing is 5-7 business days.

Updated Form – $15

Standard processing is 5-7 business days.

Expedited Request – $50

Guaranteed turnaround is 3 business days.

Forms can generally be completed within these timeframes when your medical record contains all of the information needed. If additional clarification is required, MedRecs Management may contact your provider.

Submitting forms outside of the MedRecs Management process may cause delays.

The Following Forms are Handled Internally and Should NOT be Submitted through MedRecs Management

• Disabled Parking Permit Forms

Forms We Do NOT Complete

Please note that there are certain forms and paperwork requests that New York Spine & Wellness Center does NOT complete.

The following forms should NOT be submitted to MedRecs Management or brought to a New York Spine & Wellness Center office:

• MAS 2020 Transportation Form
• Call-A-Bus Form
• NYS DMV Tinted Window Medical Exemption Form
• Physician’s Order for Personal Care Form
• Student Loan Forms (for individual who is not the patient)

Please do NOT submit the forms listed above. These forms will not be completed by MedRecs Management or New York Spine & Wellness Center.
