Mobile Check-in

3 days in advance of your scheduled office appointment, you will be sent a secure link to your phone and/or email. As soon as you receive the text or email you can immediately click on the link to start your pre check-in from the comfort of your home.

Step #1
Follow the prompts, answering all questions and completing any forms required. Have your license and insurance card on hand as you will need these to complete the Pre Check-in. You will be asked to verify and/or update information, sign any necessary forms and pay co-pays. The first-time you use Pre Check-in, the system will require you to complete forms that you may have previously completed in the office. Your future Pre Check-ins will become much more efficient! Please note, forms will still be required to be renewed on a yearly basis.

Please make sure you answer and complete all required information. Any incomplete information will delay step #2 of the process.

Step #2
When you arrive at the office, scan the QR code, (located in the office entrance) with your mobile phone camera which lets us know you have arrived in the office. Once completed have a seat in the waiting area to be called. If you neglected to complete or did not answer portions of the pre check-in (step #1), you will be directed back to the link

If you currently are not a member of the Follow My Health Portal, please make a request for an invite at your next scheduled office visit.
